First, a word about the names, because they mislead. A donut is also a cutout cushion — so is a coccyx cushion, and so is ours. All three remove material; what separates them is where the opening is and how many there are. A donut takes out one round hole in the middle. Ours takes out two, one under each sit bone. Calling one “the cutout” and the other “the donut” is shorthand, not a real category line — and the shorthand is what sends people to the wrong shape.

Everything about how the two shapes feel under you — at minute five, and at hour three — follows from what each one removes.

What a donut cushion actually does

A donut cushion is a ring of foam, gel, or inflatable rubber with a hole in the middle. The design goal is total removal of central contact: whatever sits directly over the hole bears no pressure at all, because there is nothing there to press back. That is a real capability, and no cushion that keeps material in its center can match it.

But your weight does not change when the center disappears. The load transfers to the ring around the perimeter, and everything a donut does over the next hour follows from that.

The ring problem

Pressure is force divided by area. Remove the center of a seat and the force stays the same while the contact area shrinks to a rim a few inches wide. Concentration is not a side effect of the donut shape — it is the shape.

For the first stretch of sitting, the trade-off feels good. The spot you were protecting is floating, and the rim contact is fresh. The problem is time. Tissue under sustained load compresses, and a narrow ring pressing into the underside of the thighs and the outer glutes puts weight on parts of you that never carry it in normal sitting. Over a long sit, the rim becomes the new pressure point.

This is the part that rarely makes it into product descriptions: a donut does not eliminate pressure. It moves the same pounds onto less area, on a delay. Whether that delay matters comes down to a single question: how long you sit.

What twin cutouts do differently

When you sit, most of your seated load funnels through the ischial tuberosities, the two sit bones at the base of the pelvis. They are the reason a hard chair hurts in two specific places rather than everywhere at once; sit-bone pain when sitting covers the mechanics.

Stability and height

Shape also decides how a cushion behaves when you move. A ring puts a hole in the middle of your base of support, so you balance on the rim, and the ring tilts when you shift. Inflatable donuts add a second wobble, because air moves away from wherever you press. Most donuts are also tall, and since you settle partway into the hole rather than onto a surface, your final sitting height is hard to predict.

When a donut is the right object

If you need zero contact across the whole central area, not reduced pressure but none, a donut delivers that and a cutout cushion does not. Donuts also make sense for short sits. The ring problem is a duration problem, and in twenty or thirty minutes it never has time to develop. Geometry only helps if it lines up with yours. Location matters as much as width — and so does the job: if your problem is sitting angle rather than pressure points, a wedge is the relevant category, not either shape here.
